How do I replace a lost title?

 

Complete form MV 902 .

You must be the current owner of the vehicle to apply for a duplicate title certificate. Your name must appear as the owner on the DMV records unless your name changed, the owner is deceased, or the applicant has power of attorney.

If you are an auto dealer and you have permission to receive a title on behalf of the owner you can complete a Dealer Authorization Form signed by the owner along with proof of ID.  The dealer letterhead must appear on the top of the form.

To complete the transaction you will need the following information:

Name of Owner / Date of Birth / Address / Year / Make / Vehicle I.D. #

If you address has changed since the title has been issued please indicate so by checking off the change of address box.

If you are releasing a lien from your duplicate title please include your original lien release  (MV 901) or affidavit from lien holder.

If the owner is deceased: Check the "Owner is Deceased" box on the MV 902 in section 1.  Enter the name and date of birth of the deceased. In the C/O space provide your name on the MV 902.   A copy of the Death Certificate or Letters of Testamentary or Letters of Administration are required.   

To transfer ownership of the vehicle to anther person, the spouse or guardian must complete form MV 349.1 (Affidavit for Transfer of Motor Vehicle).  The form must be notarized.  To transfer the ownership of snowmobile a boat, or a trailer, use form MV-349.2. (Affidavit for Transfer of (Snowmobile / Boat / Trailer).  The spouse or guardian also gives the new owner a photocopy of the death certificate.  The DMV accepts a photocopy of the death certificate. 

Proof of I.D. is required with all duplicate transactions.
