Ignition Interlock Device Installation Confirmation

The official confirmation form used by installers to certify that an Ignition Interlock Device (IID) has been placed in a vehicle.

A copy of this confirmation must be sent to the driver's monitoring agency (often probation) within 3 business days of installation.
