Claim for Sales and Use Tax Exemption - Title/Registration Motor Vehicle, Trailer, All-Terrain Vehicle (ATV), Vessel (Boat), or Snowmobile
Use this form to claim an exemption from sales tax when registering a motor vehicle, trailer, boat, or snowmobile.

Statement of Transaction – Claim for Credit of Sales Tax Paid to Another State
Used when a New York resident purchases a vehicle out-of-state, registers the vehicle in New York State and applies for credit for the sales tax paid in the other state.
MV-900 Notice of Lien
Filed by lienholder to record a lien on a vehicle title. Liens can be filed with the New York State DMV for all titled vehicles, boats, and manufactured homes (MFH). The lien filing fee is $5.00 per lien filed and is paid by the lender.

MV-51B - Statement of Ownership for Non-Titled Vehicles, Boats, Snowmobiles and All-Terrain Vehicles
NY DMV form MV-51B is a statement of ownership for vehicles that don't have a title, like ATVs, snowmobiles, and older cars. You use it to register one of these vehicles, transfer ownership, or if you've lost the original proof of ownership.
MV-1441.3 - Certification of Lost License, Permit or Plates
This form is used to inform the New York Department of Motor Vehicles (DMV) that you are unable to surrender your driver's license, learner's permit, non-driver ID, or license plates. This is typically necessary when your driving privileges have been suspended or revoked.
MV-950 - Affirmation of Repossesion and Bill of Sale
This form is used by a lender to affirm that a vehicle has been repossessed in accordance with New York State law. It also serves as a bill of sale if the lender is selling the vehicle. The form includes sections for vehicle details, an odometer disclosure statement, and a damage disclosure statement.
