The New York DMV DS-7 form, titled Request for Driver Review, is a legal document that allows concerned citizens to report individuals who exhibit unsafe driving behaviors or medical conditions that may impair their ability to operate a motor vehicle. This reporting mechanism is a key component of the state's driver re-evaluation program, which aims to enhance road safety by proactively addressing potential hazards. The form requires the reporter to provide specific details about the driver in question, including their identity, vehicle information, and detailed descriptions of observed incidents or behaviors. While the DMV keeps the identity of the person filing the report confidential in response to Freedom of Information Law (FOIL) requests, the submission can lead to serious legal consequences for the reported driver, including mandatory re-examinations or the suspension and revocation of their driver license.

Upon receiving a DS-7 form, the DMV Testing and Investigation Unit evaluates the report for "reasonable grounds," which must be based on specific driving performance issues rather than age alone. If the report is deemed valid, the DMV may initiate a formal review process that includes an interview with a license examiner and potentially a series of tests, such as vision, written, and road skills evaluations. The driver may also be required to provide a physician's statement (Form MV-80) to certify their medical fitness to drive. Failure to appear for these evaluations or to provide the necessary medical documentation typically results in immediate license suspension until the driver can demonstrate they are qualified to safely return to the road.
