The NY DMV DS-885 form, titled Article 19-A Bus Driver Add/Drop Notice, is a mandatory document used by motor carriers to notify the Department of Motor Vehicles when they hire, reinstate, or terminate the service of a bus driver. Governed by Article 19-A of the Vehicle and Traffic Law, this form ensures the DMV can accurately track the safety records and employment history of commercial passenger drivers. Carriers must submit the notice within 10 days of the change in employment status, either electronically or via mail/fax to the Bus Driver Unit.

The form is divided into sections for carrier information and specific columns for "Adds" and "Drops." When adding a driver, the carrier provides the individual's name, license client ID, and date of birth. When dropping a driver, the carrier must specify the reason, such as a leave of absence, resignation, or disqualification. If a driver is disqualified due to failing required biennial road tests, oral/written exams, or medical evaluations, the carrier must indicate this on the form and attach the relevant failed documentation. Additionally, the DS-885 must be retained in the driver's permanent 19-A file maintained by the motor carrier.
