FS-25

The New York State Department of Motor Vehicles (DMV) Form FS-25, titled Request and Reply for New York Insurance Information, is used to identify the insurance provider for a vehicle registered in New York that was involved in a specific motor vehicle crash. This form is primarily utilized by accident victims, attorneys, and law enforcement to facilitate insurance claims when coverage information is not immediately available.

To process a request, the applicant must provide the date of the crash, the vehicle's year and make, and either the registrant’s name or the vehicle's plate number. The submission requires a $10.00 search fee and must be accompanied by a copy of the police or motorist crash report (Form MV-104 or MV-104P). Once submitted to the Certified Document Center in Albany, the DMV will either provide the insurance company name and policy number or notify the requester if no insurance was in effect, in which case the matter may be referred to the Insurance Services Bureau for further action or license revocation proceedings.
