HAZ-44

The NY DMV HAZ-44 form is the official application for commercial drivers to obtain or renew a Hazardous Materials (HazMat) endorsement (H or X) on their Commercial Driver License (CDL). To qualify, applicants must be at least 21 years old, be a U.S. citizen or lawful permanent resident, and complete specific entry-level driver training (ELDT) before taking a mandatory 20-question written knowledge test. A critical component of the application process is a comprehensive fingerprint-based background check conducted by the Transportation Security Administration (TSA) and the state to identify potential security threats or disqualifying criminal histories, such as terrorism, murder, or certain felonies.

Once approved, the HazMat endorsement is generally valid for five years, while the CDL itself may be valid for eight.
