IRP-6A

Form IRP-6A, also known as the International Registration Plan Schedule A & C

- Part 5, is a supplemental document issued by the New York State Department of Motor Vehicles for carriers managing large fleets under the International Registration Plan (IRP). It serves as an extension of form IRP-6, providing additional space to record vehicle-specific data when adding multiple units to a new or existing fleet, or when making changes to multiple vehicles simultaneously. The form requires detailed technical specifications for each vehicle, including the Vehicle Identification Number (VIN), unladen weight, number of axles, fuel type, and the maximum desired gross weight for operation across various jurisdictions.

Beyond vehicle specifications, the document facilitates the "Registration Authorization" process, allowing a vehicle owner to formally grant permission for a different individual or entity to register the vehicle in their name. It also includes sections for identifying the entity responsible for vehicle safety, requiring their legal name, US DOT number, and Taxpayer Identification Number. By signing the form, the registrant certifies the accuracy of the information under penalty of perjury and acknowledges that false statements can lead to criminal prosecution and the suspension or revocation of vehicle registrations.
