IRP-95

The form is divided into two primary sections: Section 1 collects the primary registrant's information, including their name, date of birth, and specific vehicle details like the make, year, and Vehicle Identification Number (VIN). If the vehicle is owned by a corporation, the authorized individual must also provide their title.
