The NY DMV MV-109 form, titled Information and Certification for Vanpool Plates, is a dual-purpose document used to apply for or renew specialized vanpool registration in New York State. To be eligible, a vehicle must have an adult seating capacity of 6 to 14 passengers (excluding the driver) and be used for "vanpool" transportation—defined as the regular, prearranged transport of people to a common destination, such as a workplace, on a not-for-profit basis where fees only cover direct operating expenses like fuel, tolls, and insurance. While registration fees and local taxes remain the same as standard passenger vehicles, vanpool plates grant special privileges, including access to restricted commuter lanes and dedicated parking areas.

The application process requires the registrant to provide detailed operational data, including the vehicle identification number (VIN), specific travel routes and highways used, and a complete manifest of regular passengers with their home and work addresses. For new or exchanged plates, the form must be submitted to the Custom Plates Unit in Albany along with a registration application (MV-82) and, if applicable, a blank check for fees.
