Submitting the form initiates an investigation by the DMV Counsel’s Office and Office of Fiscal Management, with claims exceeding $1,000 also requiring review by the New York State Attorney General’s Office. If approved, the Office of the State Comptroller issues payment; otherwise, the claimant is notified of the denial in writing, typically within a processing window of four to six weeks. The release section of the form serves as a legal waiver, discharging the State of New York from further liability regarding the specific claim once it is executed.
