The NY DMV MV-278.8CDS form is the official order form used by commercial driving schools to purchase Pre-Licensing Course Completion Certificate (MV-278) books. Each book contains 50 certificates and costs $50, with payment required in full via check or money order payable to the Commissioner of Motor Vehicles. To successfully place an order, providers must be on the Authorized Signature List, stay within their authorized book limit, and ensure all required class rosters (MV-278.5N) have been submitted to their local Motor Vehicles Testing and Investigation Unit to justify the order quantity.
