The New York State Vision Test Report (Form MV-619) is a mandatory document for motorists renewing their driver license to certify they meet the state's visual acuity standard of at least 20/40 in one or both eyes. This report must be completed by a licensed healthcare professional—such as a physician, optometrist, or registered nurse—who conducts the examination and signs the form. The document is valid for 12 months, though a provider may specify a 6-month validity based on the patient's visual health.

Drivers can bypass the physical form if their test is performed by a provider enrolled in the DMV Online Vision Registry, as results are transmitted electronically to allow for immediate online license renewal. However, if a provider is not in the registry, the motorist must submit the paper MV-619 when renewing by mail or in person. Beyond standard renewals, the form is also utilized during the DMV’s driver re-evaluation and medical review programs to assess if a driver’s vision remains sufficient for safe vehicle operation following reported medical incidents or crashes. In certain cases, such as for drivers using telescopic lenses or those with vision between 20/40 and 20/70, the more specialized Form MV-80L is required instead.
