The New York State Department of Motor Vehicles Form MV-664.1 is the primary application used by residents to obtain license plates or parking permits for persons with severe disabilities. To qualify, an applicant must have a permanent or temporary impairment that limits mobility, such as legal blindness, the need for portable oxygen, the inability to walk 200 feet without stopping, or severe neurological, orthopedic, or cardiac conditions. The application requires a medical certification from a licensed healthcare provider, such as a physician, nurse practitioner, or physician assistant, though this requirement may be waived for individuals with obvious, visually identifiable permanent disabilities.

The form distinguishes between two types of parking privileges: license plates, which are issued by the DMV for vehicles registered to the person with a permanent disability, and hangtag parking permits, which are issued by local municipal clerks or police departments. Unlike license plates, parking permits can be issued to non-drivers, including children, and are valid regardless of the vehicle being used, provided the permit holder is traveling in it. While these permits are generally recognized throughout New York State, they do not exempt holders from local parking fees or regulations, except in cases where a separate Metered Parking Waiver (Form MV-664.1MP) has been granted. In New York City, the state permit is restricted to off-street parking, as the city requires a specific NYC-issued permit for on-street parking privileges.
