Form MV-664.1 is the official New York State Department of Motor Vehicles application used by residents to obtain license plates or parking permits for individuals with severe disabilities. Eligibility is restricted to those with permanent or temporary impairments that significantly limit mobility, such as the use of portable oxygen, legal blindness, the inability to walk 200 feet without stopping, or neuromuscular dysfunctions. While the DMV issues license plates for permanent disabilities, parking permits are issued by local municipal agents (city, town, or village clerks) and can be granted for both permanent (blue hangtag) and temporary (red hangtag) conditions.

The application process requires the applicant to complete personal information in Part 1 and obtain a medical certification in Part 2 from an authorized healthcare professional, such as a physician, nurse practitioner, or podiatrist. This medical requirement may be waived by the issuing agent if the disability is obvious and visually identifiable, such as the loss of a limb. Once issued, these permits and plates allow for parking in designated reserved spaces marked with the International Symbol of Access across New York State and in many other jurisdictions. However, they do not exempt holders from general parking fees or regulations, and in New York City, the state permit is only valid for off-street parking lots rather than on-street parking. Abuse of these privileges, such as using a permit when the disabled person is not present or filing a false application, is subject to significant fines and legal penalties under the Vehicle and Traffic Law.
