The NY DMV MV-664.1MP form is the official application for a Metered Parking Waiver for Persons with Severe Disabilities. This waiver allows eligible individuals to park at any metered space in New York State (excluding New York City) without paying the meter fee, provided the holder is driving the vehicle, is not accompanied by anyone able to pay the meter, and observes all posted time limits.

To qualify, an applicant must be a New York State resident with a valid driver license and a permanent, severe disability as defined by state law. Specifically, the disability must severely limit fine motor control in both hands, the ability to reach a meter due to the use of an ambulatory device, or the ability to reach a height of 42 inches from the ground. The application requires a medical certification from a physician, physician assistant, or doctor of osteopathy, and must be submitted to a local issuing agent, such as a city, town, or village clerk, rather than the DMV.
