MV-75

The NY DMV MV-75 form, titled Notification of Military Service, is used by active-duty military personnel to formally notify the Department of Motor Vehicles of their service status to maintain their driving privileges. Under New York State law, the expiration date of a driver license is automatically extended for the duration of active service and remains valid for up to six months after discharge or separation. Filing this form ensures that the DMV updates the individual’s driver record, preventing the license from being purged or penalized for expiration while the member is deployed or stationed away.

The MV-75 is a two-part document that requires the applicant to provide their full name, current address, license number, and date of entry into military service. To complete the process, the applicant must mail Part 1 to the DMV License Production Bureau in Albany and keep Part 2 physically with their driver license as proof of the extension. While the law provides an automatic extension, failing to file the MV-75 can result in difficulties when attempting to renew the license post-discharge, as no more than 30 months may pass from the date of separation to renew an expired license without retesting. There is no fee to file this notification.
