When it's required: You need this form if you lose both of your license plates (or your single plate, if you drive a motorcycle) to safely surrender your vehicle registration to the DMV and request replacements.
Fee Waivers: If the MV-78B indicates that your plates were stolen or destroyed as a result of a crime, the DMV will waive the standard replacement plate fees ($25 for a set of two; $12.50 for a single plate).
