MV-80

The Physician's Statement (Form MV-80) is a medical report used by the New York State Department of Motor Vehicles (DMV) to evaluate a driver's fitness to safely operate a motor vehicle when a medical condition other than loss of consciousness is reported. This document is typically required during the driver license application or renewal process if a medical condition is disclosed, or as part of a driver re-evaluation program following a police report, accident, or reliable report from a third party.

The form must be completed by a licensed healthcare provider—such as a physician, physician assistant, or nurse practitioner—based on a physical examination conducted within the last six months. It requires the professional to detail the patient's medical history, current medications, and their clinical opinion on whether the patient's condition interferes with safe driving. If the medical professional indicates the patient is unfit or if the form is not submitted in a timely manner, the DMV may suspend the individual's driving privileges until an acceptable statement is provided.
