For telescopic lens wearers, the form establishes rigorous criteria, including a minimum training period of 60 days and specific acuity requirements through both the telescopic and carrier lenses. The MV-80L must be based on an examination conducted within 60 days of submission and must be sent directly to the DMV Medical Review Unit in Albany for approval before the individual can renew or maintain their driving privileges.
