NDR-1

The NY DMV NDR-1 form, "Individual's Request for National Driver Register (NDR) File Search," is used by individuals to request a search of the National Driver Register to determine if their driving record contains entries from any U.S. state. The NDR is a federal database maintained by the National Highway Traffic Safety Administration (NHTSA) that tracks "problem drivers" whose licenses have been revoked, suspended, canceled, or denied, or who have been convicted of serious traffic violations like DWI.

The form requires the applicant's full identification details, including name, date of birth, sex, social security number (optional), and driver license information. To be processed, the request must be signed in the presence of a DMV representative or notarized. Once submitted, the NDR provides a report directly to the individual indicating their eligibility status—either "Eligible" or "Not Eligible"—and identifies any reporting states. If a record is found, the report will include the reason for the license sanction and the contact information for the state that reported the data, as individuals must resolve outstanding issues directly with the reporting state to regain driving privileges.
