The Physician's Statement for Medical Review Unit (Form PD-2) is a critical document used by the New York State Department of Motor Vehicles to evaluate a driver's medical fitness to operate a motor vehicle safely. It is required for individuals reporting new or existing medical, physical, or mental conditions—such as epilepsy, dementia, sleep disorders, or heart conditions—that may impair driving ability. The form consists of a section for the driver's personal information and a comprehensive medical evaluation page that must be completed by a licensed physician, physician assistant, or nurse practitioner based on an examination conducted within the last 120 days.

The medical professional must detail the diagnosis, symptoms, frequency of episodes, and any history of loss of consciousness or motor vehicle accidents related to the condition. Crucially, the provider must offer a formal medical opinion on whether the patient's condition prevents safe driving, requires further DMV evaluation, or will not interfere with vehicle operation. Based on this clinical data, the DMV Medical Review Unit determines whether to allow the individual to retain their driving privileges, require periodic medical updates, or suspend their license to ensure public safety.
