VS-19

The Statement of Ownership and/or Permission to Use Place of Business (Form VS-19) is a legal document issued by the New York State Department of Motor Vehicles (DMV) for businesses regulated by the Division of Vehicle Safety Services, such as dealerships, repair shops, and inspection stations. It is primarily used to certify that a business owner either owns the property where the business is located or has formal permission/a lease to operate there. This form is mandatory when applying for an original facility certificate or when reporting a change of business location in conjunction with form MV-253G.

The form requires detailed information regarding the business entity, the property owner, and the terms of the land use. Applicants must provide business contact details, zoning confirmation, and, if leasing, must demonstrate a lease term of at least six months; if a lease expires within six months, a letter of intent to renew from the landlord is required. The document also includes a certification section where the applicant affirms the truth of the information under penalty of law, as false statements can lead to the denial or revocation of business certificates. While it serves as proof of a physical location for DMV registration, the business remains responsible for complying with all other local and state regulations.
