The NY DMV MV-15GC form, also known as the General Consent for Release of Personal Information, is a legal document that allows a motorist to authorize the New York State Department of Motor Vehicles to disclose their personal information to a specific requester. This form serves as evidence of consent under the federal Driver’s Privacy Protection Act (DPPA), which otherwise restricts the release of sensitive data such as social security numbers, driver license numbers, and medical information.

To be valid, the motorist must complete the form by providing their name and the name of the record requester, and the document must be signed in the presence of a notary public. Once notarized, the form is typically submitted alongside a records request (such as form MV-15) to facilitate the release of records that the requester would not otherwise be permitted to access.
