This specialized version of the lien filing form allows the $5.00 filing fee to be automatically deducted from the lienholder's escrow account rather than requiring a separate payment. To ensure successful recording, all information must be typed, and the owner's name must exactly match the name on the existing title or driver license.

The form requires comprehensive details, including the Vehicle Identification Number (VIN) or Hull Identification Number (HIN), owner and co-owner identification, and the lienholder’s specific DMV-assigned filing code and account number. Both the vehicle owner and the lienholder must provide original signatures; by signing, the owner acknowledges the lien and agrees that a new Certificate of Title showing the lienholder's name will be issued. If a title has already been issued for the property, the original document must be submitted along with this form to the DMV Title Bureau in Albany for processing.
